IT Band Syndrome

What is IT Band Syndrome?

→ IT Band Syndrome (Iliotibial Band Syndrome) is a common overuse injury that causes pain on the outer side of the knee or thigh. It occurs when the iliotibial band, a thick band of connective tissue running from the hip to the shin, becomes tight or inflamed due to repetitive friction over the knee joint. It is often seen in runners, cyclists, and athletes.


Causes

→ Repetitive knee bending (running, cycling, jumping)

→ Tight iliotibial band or tight hip muscles

→ Weak hip and gluteal muscles

→ Improper running or walking technique

→ Sudden increase in training intensity or distance

→ Poor footwear or worn-out shoes

→ Running on uneven or sloped surfaces

→ Leg length differences or biomechanical imbalances


Symptoms

→ Pain on the outer side of the knee

→ Pain that worsens with activity (running, climbing stairs)

→ Clicking or snapping sensation near the knee

→ Tenderness over the outer knee or thigh

→ Pain that improves with rest but returns during activity

→ Swelling in some cases

→ Difficulty continuing physical activity


Diagnosis

→ Clinical examination by a doctor or physiotherapist

→ Medical history and activity review

→ Physical tests (checking tenderness and movement)

→ Ober’s test (to assess IT band tightness)

→ Ultrasound or MRI (to rule out other injuries)

→ Gait and posture analysis


Treatment Management

→ Rest and avoiding aggravating activities

→ Ice therapy to reduce pain and inflammation

→ Anti-inflammatory medications (NSAIDs)

→ Stretching exercises for IT band, hips, and thighs

→ Strengthening exercises (especially hip and glute muscles)

→ Physiotherapy and manual therapy

→ Foam rolling to release tight muscles

→ Corrective footwear or orthotics

→ Gradual return to activity with proper warm-up and technique

→ Corticosteroid injections (in severe cases, rarely needed)
